1. The Qualities of Massage

1.2 Rolling, it means use thumb, figure and back to help by using strength press to the muscle. It make them move or rolling in circle.

        มีความเชื่อว่ารากฐานของการนวดแผนโบราณมาจากประเทศอินเดีย โดยหมอชีวกโกมารภัจจ์แพทย์ประจำราชวงศ์สักยะ และแพทย์ประจำองค์พระพุทธเจ้าเป็นผู้ริเริ่มขึ้นในสมัยพุทธกาล ได้แพร่หลายเข้ามาสู่ประเทศไทยเมื่อใดนั้นไม่ปรากฏหลักฐานแน่ชัด พบว่ามีการนวดกันเฉพาะในวัง โดยจะนวด แต่พระเจ้าแผ่นดินหรือข้าราชการชั้นผู้ใหญ่เท่านั้น ต่อมาภายหลังจึงเริ่มเผยแพร่สู่บุคคลทั่วไป

        สำหรับในประเทศไทยนั้น หลักฐานทางประวัติศาสตร์ที่เก่าแก่ที่สุดชิ้นหนึ่งคือ ศิลาจารึก สมัยพ่อขุนรามคำแหง บนศิลาจารึกนั้นมีรอยจารึกเป็นรูปการรักษาโรคโดยการนวด และหลักฐานสำคัญอีกชิ้นหนึ่ง เป็นบันทึกจดหมายเหตุของราชทูตฝรั่งเศส ไซมอน ลา ลู แบร์ ในรัชสมัยของสมเด็จพระนารายณ์มหาราชแห่งกรุงศรีอยุธยา พ.ศ. 2204 กล่าวถึงการนวดเพื่อรักษาโรคของไทยว่า “ในกรุงสยามนั้นถ้าใครป่วยไข้ลง ก็จะเริ่มทำเส้นสายยืด โดยให้ผู้ชำนาญในทางนี้ขึ้นไปบนหลังแล้วใช้เท้าเหยียบ กล่าวกันว่า “หญิงมีครรภ์มักใช้ให้เด็กเหยียบ เพื่อให้คลอดบุตรง่าย” ในสมัยของสมเด็จพระนารายณ์มหาราช จัดได้ว่าเป็นยุคทองของการแพทย์แผนไทย ถึงกับขนานนามวิชาการนวดว่า “หัตถศาสตร์การนวดไทย” ซึ่งถูกจัดให้อยู่ในทำเนียบศักดินา ทำเนียบศักดินาเป็นทำเนียบที่ใช้สำหรับเจ้าฟ้าพระมหากษัตริย์เท่านั้น

        ประมาณ พ.ศ. 2300 ได้เริ่มมีการเขียนตำราเกี่ยวกับการนวดไว้ในใบลาน โดยจารึกไว้เป็นภาษาบาลี

        พ.ศ. 2310 กรุงศรีอยุธยาถูกพม่าเผาจนเสียหายอย่างหนัก ตำราเกี่ยวกับการนวดเหล่านี้จึงสูญหายไปด้วย เหลือเพียงบางส่วนเท่านั้น จึงได้มีการประมวลความรู้เกี่ยวกับการนวดแผนโบราณอย่างเป็นทางการ

        ต่อมาในรัชสมัยของพระบรมไตรโลกนาถมีการแบ่งส่วนที่ดินให้เป็นศักดินาแก่หมอนวดเช่นเดียวกับข้าราชการสมัยอยุธยา สมัยกรุงรัตนโกสินทร์ การแพทย์แผนไทยยังสืบทอดรูปแบบมาจากสมัยอยุธยา แต่ผู้มีความรู้บางส่วนหายสาบสูญไป เหลือเพียงหมอพระและหมอกลางบ้านที่อยู่ตามหัวเมืองต่าง ๆ จึงสามารถ ระดมความรู้จากชนกลุ่มนี้ได้

        ในสมัยพระบาทสมเด็จพระพุทธยอดฟ้าจุฬาโลกมหาราช รัชกาลที่ 1 ทรงโปรดให้ปั้นรูปฤาษีดัดตนเพิ่มเติมจนครบ 80 ท่า เป็นรูปปั้นซึ่งทำด้วยดีบุก และจารึกวิชาการนวดไทยลงบน แผ่นหินอ่อน 60 ภาพ รวมถึงรายละเอียดต่าง ๆ ประดับบนผนังศาลารายและบนเสาวัดโพธิ์ จนสมัย พระบาทสมเด็จพระนั่งเกล้าเจ้าอยู่หัว ทรงปฏิสังขรณ์วัดพระเชตุพนวิมลมังคลาราม (วัดโพธิ์)

        เมื่อ พ.ศ. 2374 โปรดเกล้าฯ ให้ประชุมรวบรวมตำราหมอนวด 60 ภาพ จารึกไว้ ณ ศาลาหน้าพระมหาเจดีย์ ด้านเหนือเขียนเป็นแผนรูปคน บอกตำแหน่งเส้นและจุดสำคัญสำหรับแก้โรคแต่ละโรคให้ประชาชนศึกษา อีกทั้งปรับปรุงรูปปั้นฤาษีดัดตนใหม่ให้เป็นเนื้อชิน ต่อมาในรัชกาลที่ 4 พระบาทสมเด็จพระจอมเกล้า เจ้าอยู่หัว ได้ชำระตำราแพทย์ใหม่ เรียกว่า ตำราแพทย์หลวง หรือแพทย์ในพระราชสำนัก เมื่อการแพทย์ แผนตะวันตกเข้ามาในประเทศไทย ตำราแพทย์หลวงจึงหมดความสำคัญจากราชสำนักไปในสมัย

        รัชกาลที่ 6 ส่วนหมอนวดแบบราชสำนักเป็น การนวดแบบดั้งเดิมที่ได้รับการถ่ายทอดจากบรรพบุรุษ

        ในสมัยรัชกาลที่ 5 พระบาทสมเด็จพระจุลจอมเกล้าเจ้าอยู่หัว ทรงโปรดเกล้าฯให้แพทย์หลวงจัดการ สังคายนาในปี พ.ศ. 2449 และแปลตำราแพทย์จากภาษาบาลีสันสกฤตเป็นภาษาไทยจัดหมวดหมู่ และทำเล่ม เรียกว่า ตำราแพทย์ศาสตร์สงเคราะห์ (ฉบับหลวง) ซึ่งมีภาควิชาหัตถศาสตร์ เรียกว่า ตำรานวดฉบับหลวง รวมอยู่ด้วย

        ในรัชสมัยของพระบาทสมเด็จพระจุลจอมเกล้าเจ้าอยู่หัวและพระมงกุฏเกล้าเจ้าอยู่หัว ผู้ที่มีชื่อเสียงในการนวดมาก คือ หมออินเทวดา ซึ่งเป็นหมอนวดในราชสำนัก ได้ถ่ายทอดวิชาทั้งหมด ให้แก่บุตรชายคือ หมอชิต เดชพันธ์ ซึ่งต่อมาได้ถ่ายทอดความรู้ให้แก่ลูกศิษย์ ความรู้เกี่ยวกับการนวดแผนโบราณนั้น เริ่มแพร่หลายและเปิดกว้างสำหรับบุคคลทั่วไป

        ในปี 2475 ได้มีการริเริ่มจัดตั้งสถานที่ขึ้นเพื่อเผยแพร่ความรู้ในด้านการรักษาโรคโดยใช้ยาสมุนไพรและการนวดขึ้นเป็นครั้งแรก ในครั้งนั้นมีผู้สนใจไม่มากนัก ต่อมาก็มีผู้สนใจมาเรียนการนวด กันมากขึ้น จนได้มีการจัดตั้งสมาคมแพทย์แผนโบราณแห่งประเทศไทยขึ้นเป็นครั้งแรกในประเทศไทย โดยมีอาจารย์ใหญ่ สัตวาทิน เป็นนายกสมาคมคนแรกและเป็นผู้ถ่ายทอดวิชานวดด้วย

        เมื่อวันที่ 30 พฤษภาคม 2505 หมอผล ถมยานิ่ม และคณะ จัดตั้งโรงเรียนสมาคมแพทย์แผนโบราณวัดพระเชตุพนฯ (วัดโพธิ์) เริ่มเปิดสอนโดยยึดหลักวิธีครูบอกต่อตามความชำนาญพิเศษเฉพาะตัว หลังจากนั้นในปี 2534 นายกำธร ตั้งตรงจิต อดีตนายกสมาคมแพทย์แผนโบราณ คิดจัดทำตำราการนวด ให้เป็นมาตรฐานการสอนการนวด จึงได้เชิญอาจารย์สอนนวดในวัดโพธิ์ทั้ง 18 ท่านมาประชุมวางแนวทางทำตำรานวดฉบับมาตรฐาน

จนมาถึงในปัจจุบันการนวดมีชื่อเสียงโด่งดังข้ามทวีปทั้งยุโรป อเมริกา หรือแม้แต่ในแถบเอเชียด้วยกัน ล่าสุดมีข่าวที่ฮังการีว่ามีการเปิดธุรกิจนวดแผนโบราณขึ้นมากมายในชื่อ Thai Tradition Massage

ประเภทของการนวด

        สถาบันการแพทย์แผนไทย ได้มีการแบ่งการนวดไทยหรือการนวดแผน

โบราณเป็น 2 ประเภทใหญ่ ๆ ได้แก่

        1. การนวดแบบทั่วไป (แบบเชลยศักดิ์) คือ สายชาวบ้าน ไม่มียศ ไม่มีตำแหน่ง มักใช้เท้าเหยียบศอกกด ดึง ดัด มีหลากหลายวิธีในการนวด มักนวดไล่จากเท้าขึ้นมา ซึ่งเป็นการนวดสำหรับสามัญชนปัจจุบันมีการเรียนการสอนนวดแบบทั่วไปตามสถาบันการศึกษา โดยผู้เรียนสามารถสมัครได้โดยตรงไม่มีการสอบคัดเลือก อัตราค่าเรียนขึ้นอยู่กับแต่ละสถานศึกษาจะกำหนดซึ่งจะมีทั้ง หลักสูตรระยะสั้นและระยะยาว การเรียนการสอนโดยทั่วไปเป็นการสอนตัวต่อตัวกับครูหรือศิษย์รุ่นพี่ โดยใช้การสาธิตและทดลองฝึกปฏิบัติ เนื้อหาการเรียนการสอนมักเป็นการเล่าประสบการณ์ของครู และสอนกายวิภาคศาสตร์ แบบโบราณบ้าง พร้อมทั้งอบรมจริยธรรมโดยถือหลักศีลธรรมเป็นสำคัญ สำหรับการเริ่มต้นเรียน อาจไม่พร้อมกัน แต่เมื่อครบกำหนดเวลาเรียนของศิษย์ ครูผู้สอนจะทดสอบ ผลการเรียนด้วยตนเองโดยให้ศิษย์ทดลองนวดครู หากทำได้ดี ถูกต้อง ครูจะออกใบรับรองของแพทย์แผนโบราณให้ แต่ถ้ายังทำได้ไม่ดี ไม่ถูกต้อง ก็จะให้เรียนและฝึกหัดเพิ่มเติมต่อไป โดยไม่ต้องจ่ายค่าเรียนเพิ่ม

        2. การนวดแบบราชสำนัก คือ การนวดที่หมอนวดมีความรู้ด้านเส้นสาย ซึ่งส่วนใหญ่รับราชการเป็นคุณหลวง คุณพระจะนวดถวายกษัตริย์ เจ้านายชั้นสูง เป็นหมอที่มีบรรดาศักดิ์ มีการเรียนการสอนสืบต่อกันมาเช่นเดียวกับการนวดแบบทั่วไป แต่อาจารย์ผู้สอนจะคัดเลือกศิษย์จากผู้สมัครเข้าเรียน ตั้งแต่การสัมภาษณ์ เพื่อศึกษาภูมิหลังของผู้จะมาเป็นศิษย์ ดูนิสัยใจคอ รูปร่างท่าทางว่าจะเรียนได้หรือไม่ถ้าเห็นว่าจะเรียนไปไม่ได้ตลอดก็จะไม่รับเข้าเรียนแต่แรก แต่หากไม่แน่ใจจะให้ผู้สมัครทดลอง ฝึกฝนไปพลาง ๆ ก่อน ถ้าผู้เรียนขาดความอดทนก็จะเลิกราไปเอง ส่วนผู้ที่ผ่านการทดสอบแล้วครูจะดำเนินการสอนเป็นขั้นตอนต่อไป วิธีการสอนจะใช้การสาธิต ฝึกปฏิบัติ พร้อมกับทดสอบผลการเรียน เช่นกัน เนื้อหาวิชาจะเริ่มตั้งแต่จรรยามารยาทในการเข้าหาผู้ป่วย หลักการนวดเบื้องต้นทั้งตัว กายวิภาคศาสตร์แบบโบราณ การวางมือในการนวดที่ตำแหน่งต่าง ๆ การใช้แรงในการนวด และระยะเวลาในการกด ปล่อยมือที่นวด ความเหมาะสมกับตำแหน่งและโรคที่จะรักษา รวมทั้งประสบการณ์ของครู ดังที่ได้กล่าวมาในข้างต้น เห็นได้ว่าการนวดทั้งสองแบบมีความคล้ายคลึงกันในด้านหลักการของการบำบัดรักษาโรคและอาการเจ็บป่วย แต่ยังมีความแตกต่างอยู่บ้างเช่นกัน

ความแตกต่างของการนวดแบบทั่วไปและแบบราชสำนัก

การนวดแบบทั่วไป

1. ก่อนการนวด ต้องพนมมือไหว้ครูก่อน 
2. ผู้นวดจะนั่งท่าใดก็ได้แล้วแต่ความถนัด 
3. ท่าทางผู้นวดจะมีความสุภาพน้อยกว่าแบบราชสำนัก
4. ผู้ถูกนวดต้องนอนบนพื้น
5. การนวดเพื่อรักษาโรคในลักษณะนี้จะสอบถามอาการก่อน
6. การนวดจะเริ่มจากเท้าขึ้นไปจนทั่วตัว

การนวดแบบราชสำนัก
1. ไม่ต้องไหว้ครู แต่ให้ไหว้ผู้ถูกนวดเพื่อขออภัย
2. การนั่งของผู้นวดต้องนั่งพับเพียบ
3. ท่าทางการนวดแบบนี้จะเน้นความสุภาพของผู้นวด (ยังแตกต่างในตำแหน่งการวางมือด้วย)
4. วัสดุที่รองผู้ถูกนวดมีความนิ่มกว่า
5. ใช้การคลำชีพจรเพื่อตรวจอาการที่ข้อมือทั้งสองข้างของร่างกาย
6. การนวดมักเริ่มจากส่วนบนของร่างกายก่อน

การนวดแผนโบราณนั้น หมอนวดจำเป็นต้องสัมผัสกับตัวผู้มารับบริการโดยตรง ดังนั้น

หมอนวดจะต้องมีศีลธรรมและมีจรรยาบรรณในวิชาชีพ ศีลของหมอนวดมี 3 ประการ คือ

1. ไม่ดื่มสุรา

2. ไม่หลอกลวง ไม่เลี้ยงไข้

3. ไม่เจ้าชู้ มีความสุภาพ ไม่แสดงกิริยาท่าทางลวนลามหรือใช้คำพูดแทะโลม

นอกจากนี้ ต้องไม่ทำการนวดในสถานที่อโคจรอื่น ๆ เช่น โรงแรม โรงน้ำชา สถานบ่อนการพนันโรงยาฝิ่น โรงพยาบาล สถานที่สาธารณะต่าง ๆ ฯลฯ แต่อาจไปนวดที่บ้านได้หากมีความจำเป็น จะเห็นได้ว่า ผู้ที่จะเป็นหมอนวดแผนโบราณได้นั้น จะต้องมีคุณธรรมและจริยธรรมประจำตัว จึงจะทำให้ผู้รับบริการเกิดความประทับใจ และมีเจตคติที่ดีต่อหมอนวดแผนโบราณ ดังนั้น ผู้ศึกษาจึงขอนำเสนอคุณลักษณะของผู้จะเป็นหมอนวดที่ดี 10 ประการ ซึ่งหมอนวดทุกคนควรจะยึดถือเป็นแนวทาง

ในการปฏิบัติในหน้าที่ที่ต้องรับผิดชอบ ดังต่อไปนี้

1. มีเมตตาจิตแก่คนไข้ ไม่เลือกชั้นวรรณะ
2. มีความอ่อนน้อมถ่อมตน
3. มีความละอาย เกรงกลัวต่อบาป
4. มีความละเอียดรอบคอบ สุขุม มีสติใคร่ครวญเหตุผล
5. ไม่โลภเห็นแก่ลาภ
6. ไม่โอ้อวดวิชาความรู้ให้ผู้อื่นหลงเชื่อ
7. ไม่เป็นคนเกียจคร้าน เผลอเลอ มักง่าย
8. ไม่ลำเอียงด้วยความรัก ความโกรธ ความกลัว ความหลง (โง่)
9. ไม่หวั่นไหวต่อลาภ ยศ สรรเสริญ สุข และความเสื่อม
10. ไม่ชอบการมัวเมาในหมู่อบายมุข

หลักพื้นฐานการนวดไทย

โรงเรียนแพทย์แผนโบราณวัตโพธิ์ (2529) ได้กล่าวถึงพื้นฐานการนวดไทย ซึ่งผู้ที่จะเป็นหมอนวดต้องทราบและต้องทำการศึกษาก่อนที่จะทำการนวด สามารถสรุปได้ดังนี้

1. ต้องรู้กายวิภาคศาสตร์ หมายถึง ต้องรู้ว่ารูปร่างหน้าตาและตำแหน่งของอวัยวะต่าง ๆ ในร่างกาย ถ้าไม่รู้อาจทำให้การนวดผิดพลาดและเป็นอันตรายได้

2. ต้องรู้สรีรวิทยา หมายถึง ต้องรู้หน้าที่ของอวัยวะต่าง ๆ ว่าทำงานได้แค่ไหน อย่างไร โดยเฉพาะข้อต่อและกล้ามเนื้อ การเรียนรู้ให้เข้าใจถึงสภาพ และหน้าที่ของระบบต่าง ๆ จึงเป็นพื้นฐานสำคัญที่ะทำให้เราปฏิบัติต่อร่างกายตนเองและผู้อื่นได้อย่างถูกต้อง เพื่อการศึกษาโครงสร้างและหน้าที่ของร่างกายแต่ละส่วน เราจำเป็นต้องแบ่งร่างกายออกเป็นระบบต่าง ๆ และระบบที่เกี่ยวข้องกับการนวดมากที่สุด ได้แก่ ระบบกระดูก ข้อต่อ และกล้ามเนื้อ ระบบประสาท ระบบไหลเวียนโลหิต และ

ระบบการหายใจ

1. ลักษณะการนวด มีวิธีการ ดังนี้

1.1 การกด มักใช้นิ้วหัวแม่มือกดลงที่ส่วนของร่างกาย เพื่อช่วยให้กล้ามเนื้อคลายตัวให้เลือดถูกขับออกจากหลอดเลือดที่บริเวณนั้น และเมื่อลดแรงกดลง เลือดก็จะพุ่งมาเลี้ยงบริเวณนั้นมากขึ้น ทำให้ระบบหมุนเวียนของเลือดทำหน้าที่ได้ดี ช่วยการซ่อมแซมส่วนที่สึกหรอได้ดีขึ้น

1.2 การคลึง คือ การใช้นิ้วหัวแม่มือ นิ้วมือ และสันมือ ออกแรงกดให้ลึกถึงกล้ามเนื้อให้เคลื่อนไปมาหรือคลึงเป็นลักษณะวงกลม

1.3 การบีบ เป็นการจับกล้ามเนื้อให้เต็มฝ่ามือแล้วออกแรงบีบที่กล้ามเนื้อ เป็นการเพิ่มการไหลเวียนของโลหิตมายังกล้ามเนื้อ ช่วยให้หายจากอาการเมื่อยล้า การบีบยังช่วยลดอาการเกร็งของกล้ามเนื้อด้วย

1.4 การดึง เป็นการออกแรง เพื่อที่จะยึดเส้นเอ็นของกล้ามเนื้อหรือพังผืดของข้อต่อที่หดสั้นเข้าไปให้ยืดออก เพื่อให้ส่วนนั้นทำหน้าที่ได้ตามปกติ

1.5 การบิด เป็นการออกแรงเพื่อหมุนข้อต่อหรือกล้ามเนื้อเส้นเอ็นให้ยืดออกทางด้านขวา

1.6 การดัด เป็นการออกแรงเพื่อให้ข้อต่อที่ติดขัดเคลื่อนไหวได้ตามปกติ การดัดต้องออกแรงมากและค่อนข้างรุนแรง ก่อนทำการดัดควรจะศึกษาเปรียบเทียบช่วงการเคลื่อนไหวของข้อต่อที่จะทำการดัดกับข้อต่อของผู้ป่วย

1.7 การตบตีหรือการทุบ การสับ เป็นการออกแรงกระตุ้นกล้ามเนื้ออย่างเป็นจังหวะเรามักใช้วิธีการเหล่านี้กับบริเวณหลัง เพื่อช่วยลดอาการปวดหลัง ปวดคอ หรือช่วยในการขับเสมหะเวลาไอ

1.8 การเหยียบ เป็นวิธีที่นิยมทำกัน ลักษณะของการนวด คือ ให้เด็กหรือผู้อื่นขึ้นไปเหยียบหรือเดินอยู่บนหลัง

2. อวัยวะที่ใช้ในการนวด (ปรีดา ตั้งตรงจิตร, 2534) อวัยวะที่ใช้ในการนวดนั้น สามารถ

ใช้ส่วนต่าง ๆ ของร่างกายนวดได้หลายอย่าง ดังนี้

1.1 การใช้มือ สามารถแบ่งได้เป็น 4 ลักษณะ ดังนี้

1.1.1 การใช้นิ้วมือ เป็นการใช้ข้อกลางนิ้วหัวแม่มือกดนวด ไม่ใช้ปลายนิ้ววางนิ้วทั้งหมดบริเวณที่จะนวดเพื่อช่วยพยุงน้ำหนักของตัวผู้นวดไว้ ส่วนใหญ่จุดที่จะนวดมักเป็นแอ่งหรือร่องที่เข้ากันได้ดีกับนิ้วหัวแม่มือ

1.1.2 การใช้ฝ่ามือ เป็นการใช้ฝ่ามือ กดคลึงบริเวณที่มีกล้ามเนื้ออ่อน ที่ไม่สามารถจะใช้นิ้วกดได้หรือใช้บีบกล้ามเนื้อโดยจับกล้ามเนื้อ

1.1.3 การใช้สันมือ เป็นการใช้สันมือออกแรงกดให้ลึกถึงกล้ามเนื้อ ให้กล้ามเนื้อเคลื่อนไปมาหรือคลึงเป็นลักษณะวงกลม ใช้สันมือสำหรับการทุบการสับ เป็นการออกแรงกระตุ้นกล้ามเนื้ออย่างเป็นจังหวะ

1.1.4 การใช้มือ เป็นการใช้อุ้งมือทั้งสองข้าง หรือข้างใดข้างหนึ่งในการดึง เพื่อยืดเส้นเอ็นของกล้ามเนื้อ เพื่อให้ส่วนนั้นทำงานได้ตามปกติ การบิด เพื่อหมุนข้อต่อหรือกล้ามเนื้อเส้นเอ็น ให้ยืดออกทางด้านขวาง การดัด เพื่อให้ข้อต่อที่ติดกันเคลื่อนไหวได้ตามปกติ

2.2 การใช้ศอก เป็นการใช้ศอกกดหรือคลึง บริเวณที่นิ้วมือกดไม่ถึง หรือจุดนั้นต้องการน้ำหนักแรงกดมาก เช่น บริเวณกล้ามเนื้อบ่า ฝ่าเท้า เป็นต้น

3.3 การใช้เท้า เป็นการใช้เท้าเหยียบ บริเวณที่มีกล้ามเนื้อหนา ๆ เช่น ต้นขา ห้ามใช้เท้าเหยียบไปด้านหลังผู้ถูกนวดอย่างเด็ดขาด เพราะอาจทำให้กระดูกสันหลังหัก หรือเกิดอันตรายต่ออวัยวะภายในได
